Where do I report my doctor for unethical practices?

I used to be on vicodon for herniated discs in my lumbar region, lower back, and some bad discs in my neck. After three months she asked me if I had signed a form to receive them I said I had not. She then takes one out and slams it down on her desk and tells me to sign and to not bother reading it. Well lo and behold I test positive for barbiturates of which I have never taken in my life. I requested another test but they flat out REFUSED to give me another test.and she has even gone as far as not refilling my other medications because I am not able to make all the appt. due to the fact that I have no car and the doctor works in Ft. Knox Army Hospital. Someone please help me
